As Op. Dr. Ali Tuncer, in order to provide you with the services we offer, we may need to collect and store your personal and health data within the limits required by the services to be provided.

The health data we need to collect in order to offer you health services is legally considered as sensitive personal data. In this context, according to the provision in Article 6, paragraph 2 of the Personal Data Protection Law No. 6698, "The processing of sensitive personal data without the explicit consent of the data subject is prohibited." INFORMATION NOTICE

This notice includes the personal data you provide to us verbally, in writing, visually, or electronically during our examination, as well as personal data transmitted to us via internet and mobile applications or electronically, or obtained in our office (e.g., test results, prescriptions, photos, etc.). The personal health data necessary for the execution of the services to be provided, including your name, surname, Turkish ID number, (if you are not a Turkish citizen, your passport number or temporary Turkish ID number), date and place of birth, marital status, gender, as well as various identity documents, address, phone number, email address, bank account number, IBAN, financial data, medical history in your clinical file, information related to past illnesses, examination data, data regarding procedures performed on you, prescription details, photos, all kinds of images, voice/video recordings, laboratory and imaging results, test results, as well as health and sexual life data obtained during the execution of medical diagnosis, treatment, and care services, data regarding private health insurance, and Social Security Institution data, are considered personal data.

The personal data categorized above will be processed for the purposes of "Managing Emergency Procedures, Ensuring Compliance with Legal Regulations, Managing Finance and Accounting Operations, Providing Physical Security, Monitoring and Managing Legal Affairs, Managing Communication Activities, Monitoring Business Activities/Operations, Managing Occupational Health and Safety Activities, Managing Post-Sale Support Services, Managing Production and Operational Processes of Goods/Services, Managing Customer Relationship Management, Managing Customer Satisfaction Activities, Organizing and Managing Events, Archiving and Storage Operations, Managing Contractual Procedures, Tracking Requests/Complaints, Providing Information to Relevant Authorities, Creating and Monitoring Visitor Records".

The processed data will be protected as a professional secret and kept confidential. It will only be shared with the competent authorities, such as the Ministry of Health (Examination Information Management System), Ministry of Finance, Ministry of Justice, and other government institutions, in accordance with current laws and regulations, upon official request. Additionally, if your treatment is to be carried out at any hospital, your personal data will be shared with the relevant hospital. It will not be shared with other third parties/institutions/organizations.

However, in situations where the confidentiality of personal medical records must be limited for public health protection, such as the obligation to report contagious diseases to the relevant authorities under Article 58 of the Public Health Law No. 1593, or in cases of legal obligation like crime reporting, we emphasize that personal data may be reported to the competent authorities in a limited manner and proportionally to the purpose.

Any voice or video recordings, photos, laboratory data, etc., recorded by us during your diagnosis and treatment may be used anonymously in scientific meetings, publications, patient information, consultations, and correspondence, provided that your identity remains confidential.

With respect to the personal data we collect, as per the Personal Data Protection Law No. 6698, the Council of Europe Convention No. 108 on the Protection of Individuals with regard to Automatic Processing of Personal Data, the European Convention on Human Rights Article 8, and Article 20 of the Constitution, you have the following rights:
